New Family Form to Enroll in Religious Education 2022/2023

CLICK HERE FOR THE LINK for new families to register online for 2022/2023. Please note this link is for first-time enrollment only. 

• If you are new to the Religious Education Program (RE), please complete this form.  Once it is received by the Religious Education Department, you will get an email from Parish Giving to go online to register for classes and submit payment.
  
  
• If your child was not baptized at St. David the King, we will need to review the original baptism certificate with a raised seal.   
  
  
• If your child is transfering from another RE program, you will need to obtain a transfer letter from that program.
